Indemnity & Health Declaration Form
I hereby declare and agree to the terms and conditions set forth below, in
consideration
for voluntarily participating in the Bike Rally ride-out event, and am fully aware
of all
associated risks to my life, vehicle and material belongings. I take sole and
absolute
responsibility for the well-being of myself and my material, property, equipment and
third parties. By agreeing to this declaration, I confirm that I have read,
understood,
and agree to be bound by all provisions contained herein.

I understand and acknowledge the inherent risks associated with riding a
motorcycle, including but not limited to accidents, collisions, falls, and
personal
injuries which may result in severe bodily harm, disability or even
death.

I hereby release, discharge and indemnify the event organizers, their
representatives, employees, volunteers, sponsors, and any associated parties
from
any and all liabilities, claims, demands, actions, costs and expenses,
whether at
law or in equity, arising out of or in connection with my participation in
the
event.

I acknowledge that I have reviewed the route map provided by the event
organizers and am fully aware of the route I will be taking during the
event.

I agree to abide by all rules, regulations, instructions and guidelines
provided by the event organizers, including those related to safety and
conduct.
I will wear appropriate safety gear, including a helmet and any other
required
protective equipment, during the entire duration of the event.

I acknowledge and agree to comply with all applicable motor vehicle rules
and regulations, as well as any other obligations pertaining to the event
and road
safety measures. It is my responsibility to hold valid RC, License, fitness
certificate, vehicle insurance policy, Pollution Under Control Certificate
(PUCC),
etc., and to promptly provide evidence of such compliance on request.

I acknowledge that I shall vigilantly handle my motorcycle and assure that
during the entire course of riding, no loss, damage or injury is caused to
the
organizers or their authorized persons.

In the event of any injury, accident or medical emergency during the
event,
I authorize the event organizers and their representatives to obtain medical
treatment and assistance as they deem necessary. I understand that I will be
responsible for any costs incurred for medical treatment and
transportation.

I will not participate in the event if I am found intoxicated or under the
influence of any drugs, alcohol, or any other substances that may impair my
judgement or physical abilities.

I grant the event organizers the right to use photographs, videos, and
other
media taken during the event for promotional purposes without any
compensation
to me.

I acknowledge that I assume all risks associated with my participation in
the event, including but not limited to those risks not specifically
mentioned in
this declaration.

This motorcycle is not meant for Moto-cross, Hard Enduro or dirt biking.
Exercising discretion while riding shall be the prerogative of the
rider.

I acknowledge that my participation in the event is non-transferable, and
I may not lend my bike to any other individual during the event.

I declare that my general health and physical ability for this activity is
good and there are no signs that portray me as unfit for this
activity.
Medical Conditions / Allergies Disclosure:
I hereby disclose any pre-existing medical conditions or allergies that event
organizers
should be aware of in case of a medical emergency. (To be disclosed verbally or in
writing at the time of registration if applicable.)
